I understand that in the course of attending Ruffing Montessori School, pictures may be taken or videos made of my child(ren) during both the normal course of a school day and also as a result of class projects.
I DO/DO NOT (please check one below) grant Ruffing Montessori School permission to illustrate, photograph, record and/or video tape my child(ren)’s likeness, voice and/or personal identification for use in Ruffing Montessori School materials or school related materials.
I release Ruffing Montessori School from any and all claims in the usage of my child’s likeness, voice and/or personal identification.
I also understand that there will be no financial or other remuneration for recording my child(ren), either for initial or subsequent usage, and hereby transfer to Ruffing Montessori School all copyright and other interests in any photographs, recordings or videotapes taken of my child(ren).
