I give permission for my child, [insert student name below], currently a student in [insert teacher name below]’s classroom, to take part in and be transported by bus to all field trips sponsored by my child’s classroom and Ruffing Montessori School during the 2022-2023 school year.
I will be given at least one week advance notice by my child’s teacher of all planned field trips. In addition, I will be notified by my child’s teacher if any such field trip requires my child to engage in any potentially hazardous activities. I agree to give written notice to Ruffing Montessori School in the event I choose to revoke my permission for my child to participate in a field trip.
I hereby release Ruffing Montessori School and the above named classroom of all liability and responsibility in case of accident in any manner connected with the referenced field trips or arising from any occurrence during the field trips from its commencement through its termination.
